Health & Wellbeing Coaches

The Health and Wellbeing Coach (HWBC) service works across all practices within the Well Up North PCN.  Each HWBC is assigned to a practice and has positive links with the practice, other agencies and Social Prescribing GP Link Workers. Health coaching is a supported self-management (SSM) intervention and is a partnership between health and care practitioners and patients.

HWBC's aim to support patients to take proactive steps to improve the way they manage their physical and mental health conditions, by guiding and prompting them to change their behaviour, so that they can make healthcare choices based on what matters to them.

HWBC spend time building trusting relationships, listening carefully to what matters, what motivates, and support patients to take action on their self-identified goals.  Once identified, HWBC motivate the patient in a non-judgemental way to improve their knowledge, confidence and skills, and helping them become active participants in their own health and wellbeing whilst treating the patient with respect and dignity.

Support is available to anyone over the age of 18 who:

  • Has a long-term conditions such as type 2 diabetes or chronic obstructive pulmonary disease (COPD).
  • those who are at risk of developing a long-term condition
  • support for issues such as weight management
  • managing chronic pain
  • living with depression or anxiety
  • Supporting with detriments of health, stress, and loneliness.
  • Support for making lifestyle changes

Our team of three HWBC's offer support over a number of sessions, face to face in practice or, over the phone and can accept referrals made through GP practice, pharmacists, physiotherapists, other professionals and practitioners within general practice and Social Prescribing Link Workers.
