GP Link Workers
The GP Link Worker (GPLW) service operates within all practices in the Well Up North Primary Care Network (PCN), staffed by four dedicated GPLWs. Each GPLW is assigned to a specific practice and maintains strong connections with community and voluntary sectors.
The core principles of the Link Working service are:
- To assist not only patients but also their family members through every stage of dementia. From initial concerns to post-diagnosis and adapting to life with the condition, we provide support and referrals to groups and organizations that can help.
- To ensure all carers have opportunities for annual reviews and are informed about available support.
- To ensure all practices are registered as veteran-friendly.
- To ensure all practices are dementia-friendly.
Who is considered a carer?
A carer is anyone, including children and adults who looks after a family member, partner or friend who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. The care they give is unpaid. When we refer to carers in this document, this is inclusive of both adult and young carers.
